Appointments are available within 1-2 weeks and urgent problems can usually be seen on the same day. We participate in the majority of insurance plans as well as offering care at reasonable discount to patients who are uninsured or out of network.
At this time, all appointments must be scheduled by phone. Appointment requests can be made through email (firstname.lastname@example.org) and one of our staff will contact you within 1-2 business days to set up the appointment. Request for an urgent appointment are made by calling the office during office hours.
Please have your insurance information available when you call so we can verify if you are in or out of network.
If you have received care at a hospital, ER, Urgent Care Center or with another provider (especially care for a problem you are scheduling an appointment for), it is helpful to request records be sent to our office prior to the appointment. HIPPA requires these requests either be made by the patient directly or by sending/faxing a medical record request form. Letting the provider know when you have scheduled your appointment can be important.
At this time, Connect Colorado (the Colorado Health Care Exchange) does not contract with private physicians, so we regret that we are out of network for these plans. We do offer significant discounts for all out of network and uninsured patients and you can ask for an estimate of the payment.
When coming in to the office, you will need to bring a picture ID and your insurance card. You need to pay any co-pays at the time of the visit. We take cash, debit and credit cards for payment but do not accept checks in the office.
Please print and fill out the following forms to the best of your ability prior to your appointment. For existing patients, an updated history is needed as well as the other forms if you have not been in the office since January of this year.
